New Patient Registration
To register with us you should live within our practice area (see map on The Practice Area page)
Come into either surgery to pick up the forms – one for each person registering.
Return with the completed forms and two types of identity, one photographic (e.g. passport), and one showing proof of address (e.g. letter from bank or building society, utility bill). If you are moving from another surgery, please contact them to obtain your NHS Numbers – we may not be able to register you without this!
You will be given an appointment for a New Patient Check, just to fill in some basic details. Once this is complete you are registered and can use all the services we offer.
